Advance Care Planning Packages

• Advance Health Directive

• Informed guidance to understand and document future health care, medical care and End-of-Life care choices

• Support to have a doctor review and sign the AHD, including making an appointment and transport to a doctors’ surgery if required

• Witness and signature by a JP Qualified

• Upload of AHD to MyGov-My Health record, Queensland Health electronic hospital record and QAS (Queensland Ambulance Service) via The State Office of Advance Care Planning

• An Advance Health Directive is included in Qld legislation and is a legal document offering a person an increased sense of control over their health care and/or end of life care

• Statement of Choices

• Support to complete a values-based document that records a person’s wishes and choices for health care and end of life care into the future. Although the Statement of Choices is not included in Qld Legislation, the content can still have a guiding effect by assisting substitute decision makers and clinicians if a person is unable to communicate their wishes

•  Form A is used for persons with cognitive capacity

• Form B is used for persons who have lost cognitive capacity to make health care and end of life care decisions

   Life Planning Services

• EPOA - Enduring Power of Attorney

• Allows a person to appoint someone they trust (an Attorney) to make decisions for either health or financial matters or both

• Wills

• Informed guidance to document and have witnessed a Last Will and Testament

• Government Forms and Claims

• Support to complete and lodge paperwork to Government agencies

• Support to complete and lodge forms and claims such as taxi vouchers and travel subsidy forms

• Personal Administration

• Support to pay bills and to attend to emails and correspondence

• Hoarding and Squalor 

• Including support for downsizing and creative de cluttering

• End of Life Care Support

• For people with no one to give support at end of life – End of Life Care Doula Support ensuring that no one dies alone

• Social Worker Services

• Living Bridge Community Care provide Social Worker Services to facilitate Advance Care Planning and Life Planning Services

• Telephone and On-line consolations are also available.

The above supports may be utilised as a single service or part of an

ongoing services package tailored to individual need.
